The County of Sonoma is pleased to invite you to respond to a Request for Proposals for Behavioral Health Housing Support Services.
The Sonoma County Department of Health Services (Health Services) is pleased to invite you to respond to a Request for Proposals (RFP) to provide intensive case management services for homeless individuals from the Joe Rodota Trail who have been placed in permanent supportive housing. The purpose of this competitive procurement is to identify a community-based organization to provide intensive case management services to occupants of the Joe Rodota Trail that have been transitioned into permanent supportive housing arrangement. The successful proposer will operate intensive case management programs designed to provide an array of intensive community-based services designed to maintain clients as successfully housed.
