Guidelines for Title 17 Submission to Sonoma County Public Health Laboratory

California Title 17 Section 2505 lists the specimens and/or isolates that must be submitted to the local public health laboratory for the local health jurisdiction (ie county) where the patient resides.  This requirement applies to testing performed by local clinical laboratories and by out of the area reference laboratories for your clinical laboratory, including but not limited to: Quest, LabCorp, ARUP, Sutter Shared Laboratories, Kaiser Regional Laboratories.

The Sonoma County Public Health Laboratory is the local public health laboratory for Sonoma County and Mendocino County and will receive specimens for residents of both counties.

Specimens for patients residing in SCPHL’s service area can be shipped to us at 3313 Chanate Rd., Santa Rosa, CA 95404, phone (707) 565-4711.

Include a test requisition for each sample submitted. Use this fillable form.
