Housing and Disability Advocacy Program (HDAP)

Are you homeless, disabled and need help applying for disability benefits?

The Housing and Disability Advocacy Program (HDAP) may be able to assist with 

• Wrap-Around Supportive Case Management and Resources 
• Medical Care Coordination
• Benefit Advocacy
• Housing Assistance

You may be eligible for services if:

• You are homeless or at risk of homelessness
• Have been seeking medical treatment for a disability and need assistance applying for disability benefits
• Have the ability to live independently with supportive services
