Community Health Workers (CHW) for COVID Response and Resilience

As of August 30, 2024, the CDC’s “Community Health Workers for COVID Response and Resilient Communities” (CCR-2109) grant concluded its third and final year of existence. As one of the 67 recipients of this grant, the Sonoma County CCR Team wishes to extend our gratitude to the all the CHW teams and subcontractors for the impactful work they have done over the last three years to make a difference in communities that were hit hardest by COVID-19.
