We are looking for volunteers to help with this year’s count! Friday, January 31, 2025 is the 2025 Sonoma Point-in-Time Count for all populations of person’s experiencing homelessness. The Count is a requirement for receiving federal homeless assistance funds and provides critical data used to understand homelessness locally and to support strategic decisions about our programs.

Sonoma County Homeless Housing, Assistance and Prevention (HHAP) Grant Program Round 5: Approved Application and Regional Action Plan for Sonoma County
The Homeless Housing, Assistance, and Prevention (HHAP) program provides flexible grant funding to cities, counties, and Continuums of Care through the California Department of Housing and Community Development to prevent and end homelessness in their regions. HomeFirst to operate the Sonoma County Continuum of Care’s Coordinated Entry System
The Coordinated Entry System in Sonoma County has changed operators from Catholic Charities of Santa Rosa to HomeFirst, a nonprofit agency based in Santa Clara County. The change in operators was approved by the local Continuum of Care Board and took effect on April 1.
