The Interdepartmental Multi-Disciplinary Team (IMDT)
The Interdepartmental Multi-Disciplinary Team (IMDT) is a collaborative of county departments, agencies, and community service providers that meets weekly to serve Sonoma County's unsheltered population. The team works to break down barriers and share information, ensuring a more coordinated and effective response.
The IMDT functions as a single team, with members addressing the specific needs of everyone receiving services. It includes various sub-teams, called "cohorts," such as SOUL and HEART. In total, the team brings together representatives from over 28 agencies, including various Sonoma County programs, city representatives, and non-profit organizations. This model acts as a scalable blueprint for how the county provides care for different populations.
